Hip dislocation is a rare occurrence in sports but can have serious implications for athletes. A systematic review of the treatment strategies and outcomes for hip dislocation in athletes was conducted to provide sports medicine physicians with the necessary information to treat and counsel patients with this injury. The review included 27 articles reporting on 145 patients.<br /><br />The studies revealed that various treatment strategies have been described in the literature, with nonoperative treatment and protected weightbearing being the most common approach for hips with concentric reduction and without significant fractures. Recurrence was reported in 3% of cases. Hip arthroscopic surgery was performed in some cases and found a 100% incidence of labral tears, 92% incidence of chondral injuries, 20% incidence of capsular tears, and 84% incidence of ligamentum teres tears. At final follow-up, 86% of patients reported no pain, 87% reported a successful return to play, and 11% showed radiographic evidence of osteonecrosis.<br /><br />Imaging for osteonecrosis is recommended, with evidence suggesting MRI at 4-6 weeks and follow-up at 3 months for suspicious findings. The review also highlighted the association between femoroacetabular impingement (FAI) morphology and athletic hip dislocation. Patients with posterior dislocation were found to have decreased acetabular anteversion, increased prevalence of a cam deformity, and higher alpha angles.<br /><br />Based on the evidence, the review recommends nonoperative treatment with protected weightbearing for hips with concentric reduction and without significant fractures, and operative intervention to achieve concentric reduction if nonoperative treatment fails. Further research is needed to more accurately characterize outcomes and determine optimal timing of postoperative MRI. Overall, the review provides valuable insights into the management of hip dislocation in athletes.
