When Is a Bankart Not Enough? Managing Subcritical Glenoid Bone Loss and Hill-Sachs Lesions in Anterior Instability.


Anterior shoulder instability with subcritical glenoid bone loss and Hill-Sachs lesions remains a challenging area in which failure to recognize bipolar bone loss can lead to recurrent instability and suboptimal outcomes. This practical, case-based webinar will review the natural history and evaluation of subcritical glenoid bone loss, assessment and management of Hill-Sachs defects including on-track/off-track principles, and a surgical framework for choosing among soft-tissue stabilization, remplissage, open Bankart repair, and glenoid augmentation. Faculty will emphasize evolving indications and technical pearls, including arthroscopic distal tibial allograft reconstruction for selected glenoid bone-loss patterns. Interactive cases, audience polling, and panel discussion will focus on the difficult clinical scenarios in which a Bankart repair alone may not be enough.

LEARNING OBJECTIVES
Upon completion of this educational activity, learners should be able to:  

1. Evaluate anterior shoulder instability with bipolar bone loss by accurately measuring glenoid bone loss and characterizing Hill-Sachs lesions using glenoid-track and on-track/off-track principles. 

2. Integrate the natural history of recurrent anterior instability and the clinical significance of subcritical glenoid bone loss with patient-, sport-, and lesion-specific risk factors when planning surgical stabilization. 

3. Apply a practical treatment algorithm to select among arthroscopic Bankart repair, Bankart repair with remplissage, open Bankart repair, and glenoid bony augmentation, including arthroscopic distal tibial allograft reconstruction, while recognizing common technical pitfalls and causes of failure.

Dr. Nicholas Lemme is an Assistant Professor of Orthopaedics in the Department of Orthopaedics at Brown University and an orthopaedic sports medicine surgeon at University Orthopedics. He completed his orthopaedic surgery residency at Brown University, followed by an Orthopaedic Trauma Fellowship at Brown and a Sports Medicine Fellowship at Rush University Medical Center.  

His clinical and research interests include shoulder instability, ACL and multiligament knee reconstruction, meniscal and cartilage preservation, and the treatment of athletic injuries. Dr. Lemme has authored or coauthored more than 90 peer-reviewed publications. His research has been recognized with the 2024 AOSSM Cabaud Memorial Award for Best Basic Science Manuscript.  

Dr. Lemme serves on the AOSSM Research Committee and as a peer reviewer for the American Journal of Sports Medicine. He currently serves as a Head Team Physician for Providence College Friars Basketball.  

Dr. Ryan Rauck is an Assistant Professor of Orthopaedic Surgery at The Ohio State University Wexner Medical Center, specializing in shoulder surgery and sports medicine. He also serves as a team physician for Ohio State football.  
Dr. Jonathan F. Dickens is a Professor of Orthopaedic Surgery, Team Physician, and Fellowship Director and Duke Orthopaedics. Colonel Dickens also serves in the United States Army Reserves as a Professor of Surgery at the Uniformed Services University of the Health Sciences (USUHS). He is the past Chief of Orthopaedic Sports Medicine and Shoulder Surgery at Walter Reed National Military Medical Center where he was the orthopaedic consultant to Office of the President of the United States and White House Medical Unit, Vice-Chair Department of Surgery USUHS, Professor of Surgery, Physical Medicine and Rehabilitation, and Director of Orthopaedic Research. Dr. Dickens has completed American Orthopaedic Association (AOA) North American Fellowship, the AOA American British Canadian Fellowship, and the American Orthopaedic Society for Sports Medicine (AOSSM) ESSKA Travelling. He is recipient of the AOSSM Excellence in Research Award twice as well as the AAOS OREF Kappa Delta Ann Donner Vaughn Award.

Dr. Albert Lin is Professor and Vice Chair of Education, Residency Program Director, Chief of Shoulder Surgery, Co-Director of the Pittsburgh Shoulder Institute, and Associate Chief of Sports Medicine at the University of Pittsburgh. He earned his undergraduate degree magna cum laude from Harvard University, his medical degree from Harvard Medical School, and completed his orthopaedic residency through the Harvard Combined Orthopaedic Residency Program, followed by fellowships in sports medicine at UPMC and shoulder surgery at Harvard and the Centre Orthopedique Santy in Lyon, France. 

Dr. Lin specializes in the care of athletes and shoulder conditions, with extensive experience in shoulder arthroscopy, arthroplasty, and complex reconstructive surgery. He is active in AAOS, AOSSM, ASES, ISAKOS, and AOA, and was inducted into the Herodicus Society and ASES Neer Circle in 2022. He co-chairs the ASES education committee, serves on ISAKOS scientific and shoulder committees, and is a founding board member of the international Rotator Cuff Study Group. He is associate editor for JBJS Case Connector, editor-in-chief of Annals in Joint, and sits on the editorial boards of KSSTA and Arthroscopy. 

Dr. Lin has published over 190 peer-reviewed papers and more than 500 abstracts and book chapters on shoulder instability, rotator cuff pathology, and arthroplasty, earning multiple awards from AAOS, ASES, AOSSM, and Arthroscopy. He has completed two NIH grants as Principal Investigator and currently serves as Co-Investigator on NIH and DoD grants studying shoulder instability. As Residency Program Director since 2022, he is deeply involved in medical education and has received several mentorship awards. He also serves on multiple UPMC leadership committees and is head team physician for Duquesne University and Pittsburgh Central Catholic High School. 

Dr. Ivan Wong, FRCSC, MAcM, Dip. Sport Medicine, FAANA, is a Professor of Orthopaedic Surgery at Dalhousie University and Co-Director of the Joint Preservation, Sports Medicine and Arthroscopy Fellowship Program. He specializes in hip and shoulder arthroscopy and advanced arthroscopic reconstruction techniques. Dr. Wong's research focuses on surgical innovation, medical education, and technologies that improve patient outcomes and surgeon training. He has published more than 200 peer-reviewed articles, delivered over 350 presentations internationally, and has trained orthopaedic surgeons, fellows, and residents from around the world.
