Innovative Strategies for Treatment of Bone Loss in the Unstable Shoulder

Course Description:

The treatment of bone loss in anterior shoulder instability presents many challenges and is evolving rapidly.

The traditional open bone block procedure has been challenged by recent developments of arthroscopic means of addressing bone loss on both the glenoid and humerus. This ICL will investigate in detail contemporary arthroscopic means of addressing attritional bone loss on both the humerus and glenoid

Learning Objectives:

  • To recognize the success of arthroscopic means of restoring glenoid bone loss employing both allogenic and autograft tissue
  • To appreciate the emerging truth that 'critical bone loss' depends on many factors - most of which can be addressed in minimally invasive fashion
  • To understand the role of remplissage and recognize its true limitations and appreciate the true complication rate of open bone block procedures
  • To appreciate the importance of bumper restoration, capsular tensioning, and repair of boney Bankart lesions in restoring stability
  • To recognize the relevance of Wolf's law and how even small fragments of bone affixed to the glenoid may hypertrophy
