The Use of Intra-articular Platelet-Rich Plasma as a Therapeutic Intervention for Hip Osteoarthritis: A Systematic Review and Meta-analysis (June 2023)

This systematic review and meta-analysis examined the use of intra-articular platelet-rich plasma (PRP) as a therapeutic intervention for hip osteoarthritis (OA). Eight studies with a total of 331 patients were included in the analysis. The results showed that PRP significantly reduced pain compared to baseline at multiple time points, with the greatest effect seen at the 1- to 2-month follow-up. PRP also significantly improved function at the 1- to 2-month follow-up. A single injection of PRP was found to achieve a significantly larger reduction in pain compared to multiple injections, a total injected dose of PRP less than 15 mL compared to 15 mL or more, and the use of a leukocyte-poor PRP preparation compared to leukocyte-rich PRP. Adverse effects associated with PRP injections were temporary and self-limiting, with no lasting effects reported. <br /><br />The authors conclude that PRP is a beneficial and safe treatment for hip OA, with reductions in pain and improvements in function observed compared to baseline. However, the evidence is of low to moderate quality, and larger, methodologically rigorous trials are needed to provide stronger evidence. Future trials should also include objective measures of cartilage health and assess the long-term efficacy of PRP. Overall, PRP holds promise as a nonsurgical treatment option for patients with hip OA who are not candidates for hip preservation surgery or hip replacement.
