Risk Factors for Failure After Osteochondral Allograft Transplantation of the Knee: A Systematic Review and Exploratory Meta-analysis (April 2023)

Osteochondral allograft transplantation (OCA) of the knee is a common procedure for treating cartilage defects. However, graft failure is a significant concern. This systematic review and meta-analysis aimed to identify risk factors associated with graft failure after OCA of the knee. A total of 16 studies consisting of 1401 patients were included. The analysis revealed several significant risk factors for failure after OCA, including bipolar chondral defects, male sex, older age, and higher body mass index (BMI). Patients with bipolar defects had a significantly higher risk of failure compared to those with unipolar defects. Male sex was also associated with an increased risk of failure. Older age and higher BMI at the time of surgery were also significant risk factors for failure. However, there was no significant evidence to support that concomitant procedures, chondral defect size, or defect location were associated with an increased risk of failure after OCA. The findings of this study provide important prognostic information for knee surgeons and can help guide shared decision-making discussions with patients. Patients with risk factors for failure may require closer follow-up and monitoring after surgery. Further studies are needed to evaluate other potential risk factors and to confirm the findings of this study.
