Chronic lateral ankle instability (CLAI) is a condition that can lead to intra-articular pathologies such as chondral or osteochondral lesions (OCLs). This study aimed to determine the incidence of OCLs in patients with CLAI. A systematic review and meta-analysis were conducted on 12 studies that included a total of 2,145 patients and 2,170 ankles with CLAI. The pooled incidence of OCLs in ankles with CLAI was found to be 32.2%. Of all the lesions, 43% were chondral and 57% were osteochondral. Among the OCLs, 85% were located on the talus and 17% on the distal tibia. The majority of talar OCLs (68%) were located medially. These findings highlight the importance of early recognition and treatment of ankle OCLs in the context of CLAI. It is recommended that physicians be aware of the high incidence of OCLs in patients with CLAI and consider additional diagnostic measures and adjuvant arthroscopy for appropriate management. The study provides important insights into the prevalence and distribution of OCLs in CLAI patients, which can aid in improving clinical outcomes and preventing further cartilage damage.
