This systematic review with meta-analysis aimed to evaluate the efficacy and safety of intra-articular injections of autologous adipose-derived stem cells (ASCs) or adipose-derived stromal vascular fractions (ADSVFs) for the treatment of knee osteoarthritis (OA). A total of 5 randomized controlled trials (RCTs) were included in the review. The results showed that both ASCs and ADSVFs significantly improved pain relief and functional improvement at 6 and 12 months compared to placebo or hyaluronic acid (HA) injections. However, the evidence for cartilage regeneration was limited due to heterogeneous assessment methods. Procedure-related knee pain and swelling were similar between the treatment and control groups, and no serious adverse events (SAEs) were reported. Subgroup analyses comparing ASCs and ADSVFs showed similar efficacy between the two treatments. Overall, the review concluded that intra-articular injection of autologous ASCs or ADSVFs without adjuvant treatment is effective in providing pain relief and functional improvement for patients with knee OA. However, further RCTs comparing ASCs and ADSVFs directly are needed to provide stronger evidence.
