Clinical Outcomes of Arthroscopic Bony Bankart Repair for Anterior Instability of the Shoulder: A Systematic Review (August 2023)

This systematic review examines the clinical outcomes of arthroscopic bony Bankart repair for anterior shoulder instability, a condition often resulting from traumatic shoulder dislocations. The review, conducted by Aaron Z. Chen and colleagues, pooled data from 21 studies, comprising 769 patients, mostly male, with a mean age of 26.7 years and an average follow-up duration of 42.7 months. <br /><br />The main findings indicate that arthroscopic bony Bankart repair significantly improves functional outcomes, decreases recurrence rates of shoulder instability, and supports a high return-to-sports rate. Postoperative evaluations frequently utilized advanced imaging, with many studies demonstrating high rates of bone fragment union, ranging widely depending on technique and follow-up duration.<br /><br />While these results are promising, suggesting that arthroscopic bony Bankart repair is effective for managing anterior shoulder instability, the authors recommend larger, prospective studies to firm up these findings. Consistent use of outcome measures and standardized definitions of recurrence and return to sports are necessary for more robust data. <br /><br />Future research should also focus on comparing different surgical techniques directly and determining the optimal bone loss threshold for selecting arthroscopic intervention. These steps will help establish clearer guidelines and improve patient outcomes in shoulder instability management.
