Clinical Outcomes After Anterior Cruciate Ligament Reconstruction in Patients With a Concomitant Segond Fracture

A recent systematic review investigated the clinical outcomes of patients undergoing anterior cruciate ligament (ACL) reconstruction who also had a concomitant Segond fracture. A Segond fracture is an avulsion fracture at the anterolateral tibial posterior of the Gerdy tubercle and is associated with ACL tears. The review aimed to determine whether the presence of a Segond fracture affects clinical outcomes after ACL reconstruction and whether surgical repair of the fracture is necessary.<br /><br />The review included 5 studies with a total of 2,418 patients. Four studies compared outcomes between patients with an unrepaired Segond fracture and those without a Segond fracture, while one study included patients who underwent ACL reconstruction and repair of the Segond fracture. The findings showed that an unrepaired Segond fracture does not significantly affect postoperative stability, risk of graft failure, or revision surgery after ACL reconstruction. The rate of graft failure was similar in patients with a Segond fracture compared to those without.<br /><br />There were no significant differences observed in other clinical outcomes, including International Knee Document Committee score, Lysholm score, Tegner activity scale, and postoperative knee laxity, between the group with a Segond fracture and the group without. The healing rate of unrepaired Segond fractures was reported to be around 90%.<br /><br />Based on these findings, the review suggests that an unrepaired Segond fracture does not have a significant negative impact on postoperative stability or risk of graft failure after ACL reconstruction. Future prospective studies may be needed to confirm these results and to further explore the outcomes of patients with combined ACL injury and Segond fracture compared to those without a Segond fracture when isolated ACL reconstruction is performed.
