CC2022: What Is the Failure Rate After Arthroscopic Repair of Bucket-Handle Meniscal Tears? A Systematic Review and Meta-analysis (May 2022)

A systematic review and meta-analysis was conducted to determine the failure rate after arthroscopic repair of bucket-handle meniscal tears (BHMTs) and compare it to the failure rate of simple meniscal tears. The analysis included 38 studies and found that the pooled failure rate for BHMT repairs was 14.8%. A total of 17 studies provided data on failure rates for both BHMT repairs and repairs of simple tears, demonstrating a significantly higher failure rate for BHMT repairs. Medial BHMT repairs and isolated repairs were found to have a higher risk of failure compared to lateral repairs and repairs with concomitant anterior cruciate ligament reconstruction, respectively. There was no significant difference in failure rates between tears in the red-red zone and tears in the red-white zone. Meta-regression analysis showed that the mean number of stitches had a significant effect on failure rates. Overall, the study provides a comprehensive analysis of failure rates after arthroscopic BHMT repair and identifies factors associated with higher failure rates. Further studies with higher methodological quality are needed to confirm these findings.
