This systematic review and meta-analysis compared the outcomes of quadriceps tendon (QT) autografts and hamstring tendon (HT) autografts in anterior cruciate ligament reconstruction (ACLR). The review included 20 studies comprising a total of 28,621 patients. The results showed that there were no significant differences in post-operative patient-reported outcome measures (PROMs), knee extensor and flexor strength, functional hop capacity, knee laxity, graft failure, or contralateral injury between QT and HT autografts. However, HT autografts were associated with better knee extensor strength. The study concluded that QT autografts are equivalent to HT autografts in ACLR, with comparable graft failure and clinical and functional outcomes observed.
