Outcome Comparison of Latissimus Dorsi Transfer and Pectoralis Major Transfer for Irreparable Subscapularis Tendon Tear: A Systematic Review (June 2022)

This systematic review compared the outcomes of Latissimus Dorsi Transfer (LDT) and Pectoralis Major Transfer (PMT) for treating irreparable subscapularis tendon tears. Twelve studies were included, with 184 shoulders in the PMT group and 85 shoulders in the LDT group. Overall, both LDT and PMT showed improvements in primary outcome (Constant-Murley score) and secondary outcomes such as pain scores, range of motion, and strength tests. LDT showed significantly better clinical outcomes than PMT in terms of Constant-Murley score, active forward flexion, abduction, and positive belly-press test rate. Complication rates and failure rates did not significantly differ between the groups. The authors concluded that LDT may be a better choice for treating irreparable subscapularis tendon tears compared to PMT. However, the study had limitations, including a small number of LDT cases and differences in patient characteristics between the two groups. Further high-quality randomized controlled studies are needed to evaluate the long-term outcomes of both surgical approaches.
