CC2022: Midterm Outcomes of Posterior Medial Meniscus Root Tear Repair: A Systematic Review (February 2022)

A systematic review was conducted to evaluate the midterm clinical outcomes of arthroscopic repair of posterior medial meniscus root tears (PMMRTs). A total of 28 studies, comprising 994 patients, were included in the review. The results showed that PMMRT repairs provided a functional benefit, with consistent improvements in clinical outcome scores. Functional outcome scores, including Tegner activity rating, Lysholm score, International Knee Documentation Committee (IKDC) subjective knee form score, and Hospital for Special Surgery (HSS) score, all showed significant improvements after the repairs. The studies also found that PMMRT repair slowed the progression of osteoarthritis but did not prevent it at midterm follow-up. Radiographic analysis revealed that 49% of patients experienced radiographic progression of at least 1 grade in the Kellgren-Lawrence (K-L) scale. MRI analysis showed that cartilage degeneration had progressed in 23% of patients at a mean follow-up of 31.6 months. The healing rates for PMMRT repairs were variable, with some studies reporting good or complete healing in 58% of patients, while others reported incomplete or failed healing. However, the effect of healing status on clinical outcomes was unclear. Overall, the study concluded that PMMRT repairs provide a clinical benefit and that further research is needed to better understand the optimal indications, techniques, and outcomes of this procedure.
