Influence of Fatty Infiltration on Hip Abductor Repair Outcomes: A Systematic Review and Meta-analysis (July 2022)

This systematic review and meta-analysis investigated the influence of fatty infiltration (FI) on outcomes after surgical repair of hip abductor tears. The study analyzed data from 4 studies with a total of 206 repairs in 201 patients. The severity of FI was assessed using the Goutallier-Fuchs grade. The findings showed that high-grade FI was associated with significantly less improvement in the Harris Hip Score (HHS)/modified Harris Hip Score (mHHS) compared to both no FI and low-grade FI. However, FI severity did not significantly influence the visual analog scale (VAS) score for pain. The results also indicated that open repair resulted in significantly greater improvement in VAS score for pain compared to endoscopic repair, regardless of FI severity. Overall, surgical intervention for hip abductor tears improved outcomes as reflected by changes in HHS/mHHS, but the presence of high-grade FI resulted in less improvement. This study provides important prognostic information for clinicians and emphasizes the need for consistent evaluation and reporting of FI severity in future studies. The limitations of the study include the small number of included studies and variations in the assessment of FI severity. Further research is needed in this area to explore the optimal treatment and management strategies for patients with hip abductor tears.
