A systematic review and meta-analysis compared the long-term outcomes of repair and meniscectomy for medial meniscus posterior root tears (MMPRTs). The study found that patients who underwent repair had significantly lower rates of radiographic osteoarthritis (OA) progression and conversion to total knee arthroplasty (TKA) compared to those who underwent meniscectomy. Repair resulted in an 83.5% reduced odds of radiographic OA progression and a 74.2% reduced odds of conversion to TKA. The study also compared postoperative International Knee Documentation Committee (IKDC) scores and found no significant differences between the repair and meniscectomy groups. The analysis included articles with a minimum mean 4-year follow-up for radiographic OA or conversion to TKA and assessed radiographic OA using Kellgren-Lawrence (KL) progression. Repair and meniscectomy cohorts were followed for an average of 64.8 months and 62.5 months, respectively, for KL progression. The study suggests that repair of MMPRTs should be considered when degenerative changes are not severe to achieve improved long-term outcomes. However, the study has some limitations, including potential publication bias and limited reporting of demographic data. Additional research is warranted to compare outcomes between different surgical techniques and investigate the impact of patient selection criteria on outcomes.
