Clinical Outcomes of Meniscal Allograft Transplantation With Respect to the Minimal Clinically Important Difference (October 2022)

Meniscal allograft transplantation (MAT) is a surgical procedure used to treat young active patients with postmeniscectomy pain. The minimal clinically important difference (MCID) is the minimum difference in patient-reported outcome measures (PROMs) that is considered clinically significant. In this systematic review and meta-analysis, the authors evaluated clinical outcome studies of patients undergoing MAT and compared postoperative improvements with established MCID thresholds.<br /><br />A total of 35 studies including 1658 unique patients were identified. Weighted mean postoperative improvements in PROMs exceeded MCID thresholds for the International Knee Documentation Committee (IKDC), Lysholm, and visual analog scale for pain (VAS pain). Subgroup analysis of patients undergoing isolated lateral MAT using fresh-frozen grafts also demonstrated postoperative improvements exceeding the MCID for the Lysholm and VAS pain.<br /><br />The results of this study suggest that postoperative improvements after MAT are clinically meaningful, as reflected by PROMs exceeding the MCID threshold. However, there were some limitations to the study, including heterogeneity among the included studies and the use of different PROMs.<br /><br />In conclusion, evaluating postoperative PROMs with respect to the MCID is important in assessing the clinical significance of MAT. The findings of this study indicate that MAT can lead to meaningful improvements in functional outcomes for patients.
