This systematic review examined the assessment and monitoring of growth disturbances after ACL reconstruction in skeletally immature patients. There was significant variability in reporting and monitoring, with low rates of preoperative screening and follow-up to maturity. The incidence of leg length discrepancy and angular deformity was low, but research quality was lacking. The review recommends preoperative skeletal age assessment and standing hip-to-ankle radiographs for baseline assessment, and postoperative radiographic monitoring using mechanical axes on standing hip-to-knee radiographs. Patient follow-up until skeletal maturity is advised. Further research is needed for improved understanding and management of these complications.
