Patellofemoral trochlear dysplasia is a complex structural anomaly that can lead to recurrent patellar instability. Most cases of dysplasia occur due to abnormal forces applied to the patellofemoral joint. Surgical stabilization is the optimal treatment approach, typically involving reconstruction of the medial patellofemoral ligament and/or medial quadriceps tendon-femoral ligament. Tibial tubercle transfer can also be beneficial in some cases. Trochleoplasty, a procedure to reshape the trochlear groove, may be necessary in rare cases where stable patellofemoral tracking cannot be restored otherwise. However, trochleoplasty carries risks to the articular cartilage and should be reserved for specific cases. Imaging techniques and 3-dimensional reproductions of dysplastic patellofemoral joints are valuable tools for surgical planning. Trochlear dysplasia is closely associated with patellar instability, and factors such as trochlear dysplasia, patella alta, increased patellar tilt, and excessive femoral rotation can contribute to recurrent patellar dislocations. Treatment strategies for patellar instability in patients with trochlear dysplasia involve medial patellofemoral complex reconstruction, tibial tubercle transfer, and, in some cases, trochleoplasty. It is important to consider individual patient factors and anatomy to determine the most appropriate surgical approach for each case. Long-term outcomes and the effects of surgical interventions on articular cartilage health require further investigation.
