ACL Reconstruction Combined With the Arnold-Coker Modification of the MacIntosh Lateral Extra-articular Tenodesis

This study aimed to compare the long-term outcomes of patients undergoing isolated anterior cruciate ligament reconstruction (ACLR) with hamstring tendon autograft versus those receiving a combined ACLR and lateral extra-articular tenodesis. The study included 165 consecutive patients treated at a single center. The results showed that patients who underwent isolated ACLR had a significantly higher risk of long-term osteoarthritis (OA) compared to those who received combined ACLR and lateral extra-articular tenodesis. Additional findings included better knee stability and lower graft rupture rates at the long-term follow-up in patients who underwent the combined procedure. The study also found that meniscectomy was the main risk factor negatively associated with OA changes. These findings suggest that combining ACLR with lateral extra-articular tenodesis provides better long-term outcomes and reduces the risk of OA.
