Video Summary
In this video, the speaker discusses meniscus replacement options for patients who have pain after a partial meniscectomy. The three categories of meniscal replacement options are segmental replacements or scaffolds, biological replacements and allograft transplantation, and synthetic replacements. The advantages of meniscal scaffolds include preserving meniscal tissue, restoring biomechanical condition, and slowing the progression of cartilage degeneration. However, there are limited indications and implantation can be challenging. The two options for scaffolds on the market are the collagen meniscal implant and ActiFit. Meniscus allograft transplantation is recommended for patients with subtotal or total meniscectomy, pain due to meniscal deficiency, and other specific criteria. Various surgical techniques, such as double bone plectate and bone bridge techniques, can be used for transplantation. Studies have shown high success rates and pain relief with meniscus allograft transplantation, with patient-reported outcomes favoring transplant over physical therapy. The speaker also mentions a new surface meniscus allograft replacement that unloads the medial compartment of the knee joint. Overall, meniscus replacement options act as a bridging procedure to provide patients with pain relief and chondral protection.
